As you peruse the checklist, consider some of the advantages of joining the Comprehensive Primary Care Plus (CPC+) program:
- Quarterly upfront payments, averaging between $15-$28 per beneficiary per month (PBPM) to help pay for care management investment.
- Performance bonuses, of up to $4 PBPM, if you meet quality-reporting requirements.
- Improved patient care and enhanced care coordination as a result of your operational improvement, increased access to care and larger staff -- elements that are baked into the CPC+ program.
- Long-term benefits under MACRA. The Medicare Access and CHIP Reauthorization Act (MACRA) of 2015 set up a new Part B payment structure, which will reward advanced alternative payment models (APMs) with a 5% payment bonus starting in 2019. CPC+ participants will be eligible for the APM payments, according to CMS.
If a shift to value-based care is what you're seeking, the CPC+ model might be a good way to start. CMS hopes so -- they plan to enroll 5,000 practices by the launch date of Jan. 1, 2017. The application process is open now; it closes on Sept. 15.
