Remove any prevailing payment issues caused by the twice-delayed HIPAA 5010 transition by thoroughly checking up on all claims and repeatedly contacting your Medicare administrative contractor (MAC) until you get an answer. While CMS maintains the majority of claims now are cleanly received and paid in the HIPAA 5010 standard, your peers, the MACs and CMS all say some complications causing payment delays remain.

Do you need to document the time spent, in minutes, for smoke cessation counseling (99406-99407)? If so, is it sufficient for the provider to write, “three to 10 minutes counseling provided” or “>10 minutes spent counseling…”? Also, are specific counseling details needed or is it sufficient for the provider to write, “Smoke cessation counseling provided?”
